The Exciting Opportunity:
* Case Management & Coordination: Oversee the management of HCP and private clients, ensuring care is tailored to meet individual needs while staying within budget limits and complying with the Aged Care Act and relevant standards
* Client & Family Relations: Conduct initial assessments and regular reviews, develop and update care plans, and act as the key point of contact for clients, their families, and other stakeholders
* Quality Standards & Compliance: Ensure the consistent delivery of high-quality services that meet all legislative requirements, including the Aged Care Quality Standards and Commonwealth Home Support Program guidelines
* Financial Oversight: Manage client budgets, monitor funding allocations, and assist clients in managing additional service requirements beyond their funding capacity

About you
* Experience: At least two years of experience in case management, particularly in health, aged care, or home care services
* Skills: Well-developed communication, problem-solving, and organizational skills. You should be proficient in using case management software, scheduling, and care coordination tools
* Knowledge: A solid understanding of the Commonwealth Home Support Program (CHSP), Home Care Packages (HCP), and relevant legislation
* Passion for Care: A genuine interest in working within the community to deliver high-quality, personalized care
