About the role We are seeking a dedicated and experienced Support Coordinator to join our team in Canterbury.
This full-time role requires you to be in the office for the first 2-4 weeks for training.
Following this period, a hybrid work model will be available.
As a Support Coordinator you will work closely with clients to effectively utilise their NDIS-funded supports, connect them with suitable service providers and allied health professionals, and monitor their progress towards achieving their plan goals.
Responsibilities include: Provide support to clients and their families in accessing and coordinating services.
Build collaborative relationships with internal and external stakeholders.
Facilitate the implementation of all supports outlined in NDIS plans.
Enhance clients skills to coordinate support and engage actively in the community Empower clients with a deeper understanding of their plans and how to implement them Be available and responsive to client needs, especially during crises.
Conduct regular progress reviews of client plans, ensuring timely and accurate reporting to the NDIS.
