Senior Housing Services Worker
A Senior Housing Services Worker will lead a team to provide case management, crisis intervention and outreach services to clients at risk of homelessness.
Key Responsibilities:
* Facilitate referrals to the program.
* Support the Team through operational supervision and secondary consultations.
* Liaise with other staff and service providers to develop a multidisciplinary approach.
* Facilitate client centred integrated care through assessment of needs and referral to other relevant Programs and services.
* Maintain required level of service hours and systems for the collection of statistics.
About You:
* Tertiary Qualifications in Social Work or related disciplines.
* Demonstrated skills and experience in housing, family work, case management and cross-sectoral collaboration.
