Shift Times: 7am - 3.30pm | 3.30pm - 11.30pm | 11.30pm - 7.30amStart Date: June 2024 Leave Entitlements: 6 Weeks Annual Leave + 2 Weeks Sick Leave + 2 Weeks Ancillary Leave + 1 RDO per MonthThe position requires a deep understanding of the unique needs and challenges faced by Indigenous communities, and the ability to work collaboratively with a diverse team to ensure the well-being of residents and community clients.Key ResponsibilitiesAs the Senior Care Coordinator Nurse, you'll lead the way in providing top-notch care for elderly Indigenous residents. Key responsibilities include: Supervising care plans, advocating for seniors, and collaborating with teams to improve health outcomes for elderly community members. Leading nursing care for residents, assisting with daily tasks, medications, wounds, and overall well-being. Respecting residents' cultures and preferences, involving them in care planning. Maintaining accurate records of residents' care, medical history, and any incident. Participating in efforts to improve care services, policies, and procedures through quality improvement initiatives. Keeping updated on best practices and aged care trends through training. Assisting in training and orienting the aged care team. Providing nursing support to the Community Home Support Program and NDIS team when needed. The Health ServiceYou will be working for an aged care facility funded by the National Aboriginal and Torres Strait Islander Flexible Aged Care (NATSIFAC) program. The facility is located in Katherine and serves up to 18 permanent and 2 respite First Nation residents. Qualifications & Certifications Required Current registration with AHPRA, with no restrictions Current NT Driver's License Current OCHRE Card Willingness to undergo a National Police Check Up to date immunisation status, including influenza & COVID-19 Skills & Experience Required At least 12 months of experience as a Registered Nurse, in an aged care setting. Demonstrated understanding of First Nations cultures, history, and health disparities.
