Your focus will be on conducting thorough intake assessments, ensuring each client’s unique choices and preferences are at the forefront. By collaborating closely with clients and maintaining strong relationships with external partners, you will facilitate seamless access to appropriate services and support, adhering to the "No Wrong Door" policy.In this dynamic position, you will contribute to a positive team environment through active participation in meetings, professional development, and innovation initiatives.
