As a Case Manager for our Sydney South Family Based Care team based in Hurstville, you will collaborate closely with children, young people, their carers, and families to create and implement case plans that strive towards achieving the permanency goals of those we support.
You'll build and maintain strong, functional relationships with both the children and their families, ensuring that every voice is heard and every need is met. Your efforts will be pivotal in supporting and monitoring placements, guiding carers in their foster care journey and helping them implement effective strategies that best support the children and young people within their care.
Responsibilities: Relevant bachelor's degree such as Social Work, Social Welfare, Psychology or similar (A qualification is desirable but not essential for Aboriginal and/or Torres Strait Islander applicants with relevant experience)Previous case management experience within an Out Of Home Care (OOHC) /Child Protection settingExperience supporting Children & Young People with disability, who have experienced backgrounds of trauma, abuse and/or neglect.Working understanding of the NSW Permanency Support ProgramUnderstanding of trauma and its effects on children and young peopleSuccessful candidates will be required to clear probity checks including National Criminal History Record Check and NSW Working with Children Check.
