The pharmacist would work with other members of the multidisciplinary team, as well as the older person (and their family/carers where appropriate), to ensure that pharmacotherapy is rational, safe, cost-effective, aligned with the person's health care goals and preferences, and manageable without excessive treatment burden. The aged care pharmacist plays a role leading and managing the efficient delivery of the aged care unit's clinical pharmacy services to the Wagga Wagga Base Hospital. The position will be required to undertake a clinical and leadership role, including support to junior pharmacists and pharmacy assistants/technicians.
