The Role: The Senior Community Health Social Worker is responsible for providing evidence-based Social Work services to individuals and communities with the aim of helping people build the capacity to self-manage and self-regulate their own health. This role is responsible for providing professional Social Work assessment, care-planning, and intervention to both clients in the community and inpatients in a District Hospital setting. You will need: Extensive Social Work experience, including demonstrated skills in psychosocial assessment, care planning and working 1:1 with individuals in a therapeutic setting. A demonstrated understanding of Social Work in Health frameworks, with knowledge of both Acute and Community Health Social Work principles. This position will be based in Launceston but required to provide Outreach to the Scottsdale Community Health Social Work service for 1 day per week. Travel will be completed in work hours with a fleet vehicle provided. A current driver's license and a demonstrated ability to work autonomously in regional or rural areas is essential. The Team: The Community Health Social Work team operates as part of the Primary Health North service in the Department of Health. We work in multidisciplinary teams in Community Health Centres and District Hospitals across Northern Tasmania, including two sites in Launceston. Community Health Social Workers provide private and confidential counselling and support to people over 18 years, as well as facilitate group education and wellbeing programs. The Community Health Social Work team actively contribute to community development and health promotion activities.
