The Role
We are looking for a Lifestyle Coordinator to join our team on a full-time basis, who is passionate about a person-centered approach to care, to join our wonderful facility in Wauchope, NSW.
The primary purpose of a Lifestyle Coordinator is to support residents to live well through the provision of spiritual care, including pastoral, emotional, and psychological support.
What You Will Do
* Liaising and working collaboratively with the Care Team and volunteers to provide holistic care aligned to individual needs of our residents
* Social support assessments and developing care plans
* Delivering care services to individuals based on their care plan
* Nurturing residents hobbies and interests
* Manage Leisure and lifestyle activities
* Conduct group activities that address mental, physical, spiritual, psychological and emotional needs.
What you will bring
* At least 1 years' experience in a similar role
* Cert 3 or 4 in Leisure and Health or similar
* Hold and maintain a NDIS Worker Clearance (or willing to obtain)
* Demonstrate strong verbal and written communication skills
