The In-reach to Residential Aged Care Clinician will work within the MyStep to Mental Wellbeing Program and provide high quality, evidence-based and time-limited psychological therapies to older persons residing in residential aged care facilities. The In-reach to Residential Aged Care clinician will work collaboratively with the consumer, their carers and family members, the Residential Aged Care Facility, along with other key stakeholders.
The In-reach clinician is responsible for the assessment and delivery of evidence based psychological treatment and clinical interventions for residents living in aged care facilities. Residents with mild to moderate anxiety and/or depression, or residents who are experiencing significant issues transitioning into residential care and experiencing adjustment disorders, or those with abnormal symptoms of grief and loss, for whom early treatment may avert the development of a mental illness, are eligible for support from the In-reach clinician. Services may be delivered either on a one on one basis or within group settings to assist consumers to achieve their goals using recovery focussed strategies.
