Post Custodial Support Worker - Aboriginal Identified
This is an Identified position for Aboriginal and Torres Strait Islander People. Aboriginality is a genuine qualification authorised under Section 14(d) of the Anti-Discrimination ACT 1977.
Key Responsibilities:
* Conduct comprehensive assessments, gather information that informs needs and their treatment care planning, risk identification response, and monitoring of substance-related risks.
* Collaborative care planning and intensive case management for clients throughout their tenure with the Post Custodial Support Program.
* Continual care planning that includes identified issues, goals, actions, responsible persons, and review timelines, addressing substance use, mental health, physical health, psychosocial, cultural, socioeconomic, legal, and other related concerns.
* Participate in regular clinical case reviews for monitoring and reflection on progress and priorities.
* Provide counselling and support to clients as outlined in care plans.
* Develop and maintain collaborative networks and partnerships with service providers, referral agents, and the Criminal Justice system, including Corrective Services.
* Provide detailed transfer of care summaries to clients and ongoing care providers.
Experience, Qualifications & Licenses:
* A Certificate of Aboriginality.
* A Current National Police Check.
* A Current Working With Children Check NSW.
* Current Driver’s License valid in Australia.
* COVID-19 Vaccination and Booster information (or evidence of a valid medical exemption).
* Immunisation records (if applicable).
* Knowledge and understanding of the impact of drug and alcohol use/misuse in Aboriginal communities and sensitivity to Aboriginal culture and history, including current issues affecting Aboriginal people.
* Demonstrated work experience in health, welfare, or drug and alcohol-related fields.
* Capacity to provide written reports, intake and comprehensive assessments, and holistic case management.
* Excellent communication skills and ability to work both independently and as part of a team.
* Demonstrated organisational and time management skills, along with strong interpersonal skills including risk identification and response.
* Experience in utilizing client software packages and Microsoft Office programs.
