Job Title: Care Manager
Your Role:
* Manage and coordinate care services to ensure the highest standards of care.
* Develop and implement care plans tailored to individual client needs.
* Liaise with clients, families, and healthcare professionals to ensure comprehensive care.
* Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals.
* Ensure compliance with all relevant legislation and organizational policies.
* Monitor and evaluate care services to continuously improve quality and efficiency.
* Monitor the budget of Home Care Packages and ensure clients understand their funding and fees.

Requirements for Success:
* Demonstrate experience in case management and client care coordination in a Community Aged Care setting.
* Strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people.
* Provide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough).
* Hold a current Police Screening Certificate.
* Provide a copy of your current First Aid and CPR Certificate.
* Hold a current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check).
* Hold a current Queensland driver's licence.
* Completed tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human services).
* Able to work in Maryborough, Hervey Bay and surrounding suburbs.
