Overview
An exciting opportunity is available for an Innovation and Implementation Leader to provide leadership, expertise and operational planning support to Community and Allied Health Directorate, to strengthen and enable their preparedness, capability and capacity as Commonwealth approved aged care and disability providers in the community setting.
In this role you will lead and contribute to the planning, development and establishment of a range of strategic community ageing and disability reform initiatives and provide expertise across the new Support at Home Program continuum with regards to Commonwealth reforms, system change.
The Innovation and Implementation Leader will develop key relationships with Eyre and Far North Local Health Network (EFNLHN) stakeholders allowing trust to be built and change management strategies to be developed. The role will collaborate with EFNLHN colleagues to develop standardised procedures, initiatives, and quality improvements to improve Commonwealth program service delivery and consumer outcomes.
