Position Summary: Alzheimer’s WA is seeking an experienced Care Coordinator with the enthusiasm, care, and compassion to support people living with dementia. This position coordinates the individualised support and services for our clients from onboarding through to review, ensuring the client’s needs and goals are met throughout their dementia journey. This role is based out of Hawthorn House in Albany. Responsibilities: Onboard new Level 1 and 2 participants and develop Agreements (and Budgets) to reflect the supports, funding and services requested, in a timely and accurate manner. Adherence to Aged Care Funding and operational manuals. Coordination and development of supports to a caseload of Level 1, level 2 and CHSP clients living with dementia in the community. Conduct risk assessments and develop care plans for Level 1, level 2 and CHSP clients and set up their support services accordingly. Assist the registered Nurses on non-clinical tasks as requested, for example, organising allied services, ordering stock and uploading documentation into Procura. Follow medication care plan as per registered Nurse’s assessment. Refer all clinical indicators to registered nurse for management. Work closely with clients and Support Workers to ensure quality service delivery. Report restrictive practices, incident reports to registered nurse and document in progress notes. Conduct annual assessment and annual reviews Level 1, level 2 and CHSP clients in line with Aged Care standards. Liaise with schedulers for provision of appropriate and timely client services. Prepare Level 1 and level 2 client support plans, budget management, and other necessary documentation for submission to My Aged Care, when clients' care levels increase, and available funds are running low. Actively participate in continuous improvement in delivery of services and processes. Innovative thinking to deliver a person-centred approach to all clients. Upload all support plans into our CRM for Level 1 and level 2 clients. Enter daily notes in CRM for Level 1 and level 2 clients. Work collaboratively with the registered nurse and House Manager to ensure operational reports are completed in time and are factual. Essential Criteria: Cert III, IV or Diploma – Community Services or related discipline. Detailed knowledge of Aged Care Standards and compliance requirements. Knowledge of CHSP and HCP. Knowledge of Dementia. National Police Clearance no older than six-months Current WA Driver’s License Working rights in Australia Fully vaccinated against Covid-19 Competence in creating and developing client care plans. Desirable: Experience in Dementia care is highly desirable. At least 4 years’ experience in Aged Care. Intermediate working knowledge of Microsoft Office applications.
