Task & Responsibilities PERSONAL SUPPORT FOR PARTICIPANTS• Contribute to the development of a safe and nurturing environment for participants.• Carry out personal duties including assisting with showering, dressing, feeding, toileting, personal hygiene, transport, recreational activities, personal encouragement.• Contribute to the development, implementation and review of individual care plans that meet the emotional, social and behavioral needs of participants.• Assist participants to develop independent living skills.• Protect and respect the rights of participants.• Safeguard the wellbeing of participants and report any concerns immediately.• Support participants to remain connected to the significant people and places in their lives.• Support participants to access information and experiences which assist them to develop a positive sense of identity.• Support participants to participate in decision making processes.• Provide services that are sensitive to the needs of Aboriginal and Torres Strait Islander. and Culturally and Linguistically Diverse participants and their families. TEAM ROLE• Provide support that reflects the organization’s values, policies and practice frameworks including the Participant-owned service model.• Work collaboratively with other support staff, case workers, service providers and families.• Undertake rostered duties including sleepovers.• Carry out household duties including cleaning, shopping, meal preparation, minor property and grounds maintenance.• Participate in team meetings, house meetings, case conferences and case review meetings as required.• Maintain records and documentation as required [including case notes, communication books, incident reports, hazard and injury log etc];• Contribute to the implementation of participant participation and feedback processes.• Ensure that sensitive participant information is kept in a secure and confidential Manner.• Comply with the organization’s administrative and financial management procedures. Qualification & Experience The successful applicant will have: • Willingness to provide personal care in a residential environment.• Ability to relate to and work with people with social, emotional and behavioral Difficulties: • Ability to implement behavior support plans for people with challenging behaviors;• Experience and understanding of OH&S issues in the workplace.• Good written and verbal communication skills; • Commitment to the purpose and values of Choices4Life Pty Ltd;• Qualifications in disability work or relevant field, or suitable experience;• Willingness to accept overnight, weekend public holiday shifts and home support shifts; OTHER JOB-RELATED INFORMATION The incumbent is required to:• Observe and actively support Choices4Life Pty Ltd’s OH&S policy and strategy.• Attend mandatory and other training as required. Good Directions is Choices4Life Pty Ltd's Service provider and asssists with staff and other GD-PODS management.
