Would you like to work in a dynamic multidisciplinary team, providing intensive mental health care within a young person’s home? Then come join our new Youth Mental Health Hospital in the Home (Y-HiTH) service!The CYMHS Y-HiTH is a new service for the North West region, based in Devonport.Our aim is to support young people (16–24-year-olds) who may be experiencing an acute episode of mental illness to receive treatment in their home and within the community, rather than being admitted to an inpatient mental health unit, or as part of a discharge plan from hospital.The Y-HiTH team will work with the young person and their family/significant others to develop an individual treatment plan that supports their recovery. With multiple home visits occurring every day and additional support via telephone and telehealth, young people will receive comparable treatment from Y-HiTH as they would from an inpatient admission.The Role:With a focus on engagement, work in collaboration with the young person, their family/significant others, to plan, implement and evaluate nursing care that is evidence-based, person-centred, and trauma-informed.Function as an active member of the multidisciplinary team (MDT) to provide care to all consumers of the service, which includes biopsychosocial assessments, therapeutic engagement, monitoring and review of mental state and response to treatments, and updates to the care plan in consultation with the young person, their families/significant others and the MDT.As a member of the service, evaluate the effectiveness of nursing strategies in meeting anticipated treatment outcomes and participate in the development and revision of organisational documentation, policies, protocols, and procedures relating to nursing-based best practice and the model of care for the Youth Mental Health Hospital in the Home.Complete all required clinical documentation, including assessments, risk assessments, transfer of care, relevant clinical correspondence, and clinical data collection, within specified timeframes, predominantly via electronic consumer records systems.You will need:Current knowledge and experience in and the ability to apply nursing principles and procedures in the delivery of consumer care in acute youth mental health in line with legal requirements and the Australian Nursing and Midwifery Council competency standards for Registered Nurses.Knowledge and understanding of the psychosocial and health needs of individuals and their application to nursing practice in the acute youth mental health setting, including an understanding of the principles of maintaining appropriate therapeutic relationships with consumers.Relevant experience in an acute Mental Health setting with demonstrated knowledge of relevant legislation including the Mental Health Act 2013.Details of appointment:Permanent full time, day worker/shift worker (fully rotational) position, working 76 hours per fortnight, notwithstanding hours to be negotiated with the successful applicant.Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.Salary: $78,179 to $105,995 per annum.
