What You'll Be DoingThe Discharge Patient Flow Concierge Administration Officer is responsible for supporting administrative processes for discharge planning across the Clinical Networks of the MNCLHD and assists patients and their families with the process of leaving hospital and transitioning back to their home or other care setting. The position holder will do this by supporting discharge processes in collaboration with the multidisciplinary team to expedite timely discharge and facilitate integration with out of hospital care and primary care services where appropriate.
This role will work in close collaboration with the Discharge Patient Flow Concierge Social Workers, with an active focus on supporting their roles to expedite timely discharge outcomes. Whilst this is a network position, it will be imbedded as part of the social work team. The social work department values a collaborative and supportive culture that respects and nurtures the contributions of each team member. The team comes with knowledge and experience with Senior staff that can support learning and development.
Provision of efficient, high level administrative services to the manager (and team as applicable) in line with current Mid North Coast Local Health District (MNCLHD) policies and procedures.
