We are recruiting for a skilled Registered Nurse to provide clinical and professional leadership of our North West regional team - based in Devonport or Burnie.

Job tasks and responsibilities Provide leadership, support, and mentoring to the regional office care partners and clinical services team, this includes but is not limited to: The induction of new care partners and clinical employees and the upskilling of existing team members.
Provide support to the Care Partner & Clinical Team in relation to the day-to-day operation and associated resources.
Provide expertise in relation to HCP Consumer Liaison with a strong focus on "HCP Consumer Onboarding Processes".
Provide support and assistance to consumers for the coordination and management of individualized, consumer-directed Home Care Packages (HCP), Short Term Restorative Care (STRC), Home Independence Program (HIP), and clinical care with a strong focus on wellness & re-ablement.
Ensure all services are delivered within the context of "The Charter of Aged Care Rights" & the Aged Care Quality Standards and other legislative requirements.
Skills and experience Registered Nurse with current unrestricted AHPRA registration.
National Police Certificate obtained within the last three (3) months.
Selection Criteria: Experience - Minimum Requirements At least 5 years of experience in a community-based or other relevant healthcare setting in: team leadership clinical governance Demonstrated ability to identify and provide a systems approach to workflow and to assess workload and allocate tasks as needed.
Experience - Highly Desirable Minimum 5 years' experience in a leadership role.
Demonstrated ability to provide leadership in a constantly evolving environment.
Experience in data-driven analysis and application to work processes.
Demonstrated high-level understanding of care coordination and service delivery functions.
Demonstrated understanding of client budget concepts and management.
An understanding of risk management principles, workplace health and safety, and continuous quality improvement processes and their application in the workplace.
Knowledge - Highly Desirable Knowledge of the Community Services sector.
An understanding of the independence model, wellness approach, re-ablement service provision, and restorative care.
Skills Excellent relationship, team building, and communication skills.
High level verbal and written literacy skills.
Demonstrated high level computer literacy with experience in word processing, calendar management software (Outlook & Google), client/patient related management systems, and scheduling systems.
High level keyboard skills are essential.
