We are looking for a suitably qualified & experienced Registered Nurse to provide exceptional Community Nursing, Hospital in the Home & Post-Acute Care Services.
Terang and Mortlake Health Service provides a broad range of inpatient, primary, residential aged care, home-based and community-based services supported by our multi-disciplinary team. Applications are invited from Registered Nurses who would like to take up the opportunity to join our dynamic Community Health team to work in District Nursing team in Mortlake and Terang. The primary role of the District nurse is to provide a person centred approach focusing on maximising functional capacity, home based acute care, improved self-management strategies and coordination of service delivery to clients in their own homes.
You must be committed to providing evidenced based practice, demonstrate an understanding of and commitment to nursing in the community setting and quality improvement. Applicants should have the appropriate skills including the ability to plan care, assess clients, and liaise with other care providers.
A current driver's license is essential, and post graduate qualifications are highly regarded.
