Focused Attention is a start-up company providing At Home Care through the NDIS, My Aged Care whilst offering Professional Services.
Each RN will have at least 10 years + of Healthcare experience and will be an integral member of the Focused Attention core management team, responsible for ensuring a consistently high quality of candidates managing interviews and offering Subject Matter Expertise and thought leadership whilst reporting to the Managing Director in a peer group management team.
The successful Registered Nurses would have knowledge and experience managing residents, to facilitate and support their continued enjoyment of life within their level of capability.Integral to this is proactive management of resident care, commitment to quality assurance, continuous improvement and high standards of work ethics, whilst ensuring compliance to Home guidelines/policies, Department regulations and WHS legislation.The role requires strong relationships to be maintained with residents and their families and friends, care staff, leisure and lifestyle coordinators and external professionals, Allity Services StaffKey Accountabilities:High Quality Resident CareDelivering a high quality of care to each resident in the Home, respecting their privacy and dignity while at all times displaying quality customer focus.Complete the initial assessment and care planning; and the ongoing management and evaluation of resident's individual care needs within the scope of practice.Implementing the nursing care plan as assessed for each resident by the health care team and the resident / representative.Reporting and recording information relevant to the residents' care needs.Showing respect to the property of the Residents, proprietor and economise where possible, but not to the detriment of Resident Care.Liaising with visiting medical officers and other health professionals and residents / relatives in the planning, implementation and evaluation of the resident's care needs.Having knowledge of and being accountable for the Duty of Care to residents of the Home.Complying with instructions from the General Manager/Care Manager and Registered Nurse in-charge regarding designated areas of duty and instructions regarding care delivery.Delivering to residents a holistic concept of care using the resident's care plan, utilising the 'Person Centred Care' approach.Aiming to continually improve standards of nursing care by revision of practices and through encouragement of teaching and learning situations for staff and self.Resident & Family RelationshipsMaintain professional standards at all times with resident and family dealings.Ensure the confidentiality of clinical and other records pertaining to each resident is not disclosed or discussed with or in front of anyone other than the people involved in the care of the resident.Be pleasant and maintain positive interaction with residents when delivering care.Maintain confidentiality of resident care needs.Participate in the resolution of resident or families concerns and refer complex matters to the General Manager.Practising public relations in communication with residents, relatives and visitors, ensuring that all enquiries, whether in person or by phone, are answered in a courteous and professional manner.ComplianceUnderstanding the policies and procedures of the residential aged care Home including the infection control and WHS practices as provided in Focused Attention's Quality Management System.Being familiar with and delivering the expected Standards as per the Aged Care Act 1997.Following guidelines in relation to compulsory reporting of alleged assault and resident absences.Complying with policies in regard to the use of electronic care planning and other organisation systems.Working within the guidelines of the Quality Management System, Infection Control, Manual Handling, OH&S principles and regulations.Accepting responsibility of working within the guidelines of nursing policies and maintaining an optimal standard of nursing practice by understanding the philosophy and policies of the Home and relevant best practice guidelines.Understanding and adhering to the medication policies of the Home as required by legislation.Supervisory and ManagementSupervising, managing and monitoring the standard of care delivered to the residents, ensuring that the care is individualised and person-centred according to resident's individualised care needs.Assessing and planning residents' care needs on an ongoing basis and updating documentation as per protocol.Displaying leadership, initiative and time management skills in the performance of duties at all times.Providing supervision, mentoring and training to AINs and PCWs.Protecting the interest of the Home in respect of industrial matters affecting nursing and other staff.Reporting to the Care Manager/General Manager all occurrences of incidents/observations/complaints related to residents, relatives and/or staff.Taking responsibility for specific portfolios and quality assurance activities as directed by the Care Manager/General Manager.Records ManagementEnsuring maintenance of all necessary medical records, including accurate, concise and precise daily and critical reports that will contribute to effective communication and support resident classification claims.Recording accurate medical records on each resident in line with the documentation guidelines and the legal issues of report writing.Ensuring that the confidentiality of medical and other records pertaining to each resident is not disclosed or discussed with or in front of anyone other than the people involved in the care of the resident.Providing and maintaining own personal details relevant to employee services and reporting changes or inconsistencies to management.Health, Safety and Environment (HSE)Conduct themselves in a manner that will not put themselves or others at harm.Report all incidents, hazards and near misses as per the AQMS requirements.Reports any changes in resident's care plans that may potentially cause an injury to yourself or others.Participate in HSE engagement programs, hazard inspections, HSE meetings and risk assessments as applicable.Complete all HSE compliance training such as and not limited to Manual Handling, Emergency Response and CPR, follow all Safe Work Practices and use appropriate equipment provided.Promote and adhere to all Health, Safety and Environment Policies and Procedures.Support the completion of the Home Safety Plan.Cooperate and participate with Injury Management Programs.Contribute to achieving or exceeding Focused Attention's HSE KPIs.Achieve or exceed individual HSE KPIs.DevelopmentEnsuring ongoing Registered Nurses status including continuing professional development requirements.Participating in the orientation program for agency staff and new employees as designed by the management of the Home.Attending and actively participating in designated staff meetings and other quality assurance activities such as audits / surveys as required.Be an active participant and contributor in all education programs as designed by and /or the Home and any other training as directed by management.Maintain minimum continuing education requirements/credits at all times to ensure you are up to date with current and progressive practices in high quality aged care.Actively participating in regular performance reviews.Encouraging a learning culture including thorough peer support.Consciously work closely with fellow colleagues (RN's & FM) to enhance your own clinical skills, knowledge and experience.Accepting timely revision of position description at the discretion of management and in consultation with staff.Completing organisational training requirements as directed and in the appropriate timeframes.Being involved in the appraisal of the work practices of the Assistants in Nursing/Enrolled Nurses and making recommendations for their education programs.Studying trends of current acts and regulations by way of formal and/or informal education programs.Other DutiesAnd any other duties, as required.Qualifications Required:Registered Nurse (Division 1)Graduate Certificate Aged Care (Desirable)Care/Clinical/Management/Clinical Leadership Qualifications (Desirable)Experience/Knowledge Required:Demonstrated experience in an Aged Care environment;Excellent communication style – able to communicate effectively with residents and their families including people from CALD backgrounds;Current valid criminal record check and statutory declaration as appropriate;Intermediate computer skills;Commitment to a high level of customer service and person-centred care;Motivated and able to take initiative;Organised and able to effectively
