MHS aspires to be a strong, sustainable and recognised health service that builds healthier communities and positive change for our Aboriginal and Torres Strait Islander families and those who call Roebourne home.MHS is looking for people who want to make a real difference in the health of Aboriginal and Torres Strait Islander families.The PositionThe Chronic Disease Nurse role is a part of the Clinic Team of Mawarnkarra Health Service.The role of the Chronic Disease Nurse is to ensure the ongoing provision of the highest standards of care to clients of the service through the provision of clinical services and health education and promotion to enhance the clinical services provided to clients of Mawarnkarra with the aim of improving health outcomes for Aboriginal people of the area.Aboriginal and/or Torres Strait Islander people are strongly encouraged to apply. (Refer Section 51 of the Equal Opportunity Act (WA) 1984).The RequirementsWork collaboratively with members of the clinical team to ensure the effective delivery of chronic disease services to clients.In collaboration with the multidisciplinary team conduct health checks on adult clients.Oversee the management of recalls and reminders for the clinic.Participate in case conferencing and coordinate and collaborate with other service providers including the Programs Services Team and external service providers.Identify and provide advice to the Senior Medical Officer and the Practice Manager on areas for quality improvement in program areas such that client outcomes are consistently improved.Identify service providers within the area that can assist the organization to achieve its outcomes, particularly in the area of referrals to and from the service and access to the service.Perform routine pathology tests, screening and other technical tasks such as ECGs, wound management and administration of medication.Work in collaboration with other staff at Mawarnkarra and take a leadership role in implementing education and health promotion for clients and the community.In collaboration with other staff provide education to clients and families on care, chronic disease management and health prevention.Provide education and training to other staff on issues related to chronic disease.Assist with other clinical duties such as immunizations.Promote Medicare billing in the service consistent with laws governing Medicare and in a manner that optimizes Medicare billing arrangements.Other RequirementsRegistered Nurse with AHPRAMinimum of 5+ years post grad experienceChronic Disease management and care experienceConsent to undergo a WA Working With Children Check and a National Police ClearanceCurrent CPR certificateAble to pass regular and random drug and alcohol testingRemuneration Benefits include:Salary of $110,916 - $148,875 p/a pro rata11.5% SuperannuationHousing and District allowances (subject to eligibility)Generous Salary Sacrifice options availableApplicationsPlease address the Skills and Experience mentioned above, in your Resume/Cover Letter.MHS may commence appointment proceedings immediately, appoint by invitation or make no appointment at all.
