About the Role:
Team Leader – Taree location. Full time contract until 30 June 2024.
The Social Futures Care Connect program is an aftercare service supporting people who have attempted suicide and focuses on empowering people to connect with both formal and practical supports to assist with recovery. The Team Leader Care Connect will support a small team in the regions of Upper Hunter and Manning to increase access and engagement with care and supports and provide encouragement and follow up to prevent suicidal and self-harm behaviour.
Desired Skills and Experience :
1. Tertiary qualifications and / or equivalent experience in human services, psychology, social work, nursing or social sciences
2. Demonstrated minimum 3 years’ experience in mental health, case management programs or similar
3. Experience in leading, supervising and supporting a team in a complex triage, assessment and referral environment
4. Understanding of recovery-oriented support and trauma informed care, with the ability to role model and implement these practices in service provision
5. Excellent written and oral communication, interpersonal and negotiation skills, with the ability to communicate sensitively and effectively with all people
6. Demonstrated well-developed organisational, time management and administrative skills with the ability to plan, prioritise and meet deadlines,
7. Demonstrated experience in maintaining professional boundaries while engaging in person-centred work
8. Ability to travel on a regular basis between Upper Hunter and Manning regions (access to company pool vehicle provided)
