Key ResponsibilitiesAs a Diabetes Nurse Educator, you will work to improve the health outcomes for Aboriginal and Torres Strait Islander people. You will work in collaboration with the multidisciplinary team to educate and assist individuals in managing diabetes. Your key responsibilities include a range of clinical, managerial, researching and administrative duties, including: Contributing to improving health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through better access to coordinated and multidisciplinary care. Advising patients on the importance of following a care plan, which may include structured support for diabetes self-management and assistance with care plan compliance. Travelling to remote communities. Dispensing pharmaceutical therapies, including the administration of vaccines and prescribed pharmaceuticals for common conditions. Working closely with all Aboriginal Health Practitioners (AHPs) to improve engagement and access by Aboriginal clients to health services and the attendance for necessary recalls. The provision of informal training, specifically support and education for AHPs, Doctors, RNs and other Health Centre support staff where relevant and necessary. Contributing to clinical reports and research activities. Qualifications & Certifications Required Current registration with AHPRA, with no restrictions Graduate Certificate in Diabetes Education and/or relevant experience providing clinical care and education to individuals with diabetes Current NT Police Check & OCHRE Card Skills & Experience Required Experience working with an Aboriginal organisation or communities will be highly regarded but is not essential A thorough understanding of the primary health care system and issues relevant to closing the gap in health outcomes for Aboriginal and Torres Strait Islander peoples. Confident in communicating, with the ability to conduct 1-on-1 and group sessions to educate clients and staff on best practice for diabetes health management.
