We respect, protect and promote human rights in our decision-making and actions.The Post-Acute Care Service supports early hospital discharge for people whose recuperation can be safely completed in their own homes, as an alternative to an extended hospital stay. Post-acute care services ensure that people who no longer need acute care are able to safely return home by providing short term care and assistance. The position will be required to deliver senior acute, post-acute and hospital avoidance services across the SCHHS.Key Responsibilities include: Deliver independent high level Physiotherapy clinical practice with infrequent need for direct clinical practice supervision. Exercise professional judgement to a complex caseload, adapting routine clinical practice based on valid and reliable evidence. Provide high level clinical advice, education and influence towards a common patient-centred vision and direction to professional operational teams and relevant stakeholders regarding service delivery. Provide leadership oversight and clinical practice supervision to less experienced Physiotherapy practitioners and students as delegated. Remuneration value up to $157,359 p.a., comprising salary between $128,169- $137,918 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4).
