About the Role Fixed term 24 hours per week until May 2026 Wimmera Palliative Care Service is a community palliative care service that supports people living with life limiting illnesses, who have complex symptoms, and their families and carers.
Our team supports patients at home, in aged care facilities and while in hospital.
The Wimmera Palliative Care team works with the patient's local doctors, nurses and allied health care team to provide care.
The purpose of this position is to provide palliative care nursing clinical support and act as an expert resource to clients and their carers and primary care providers across the Wimmera region.
Nurses also provide bereavement support to carers and contribute to raising the profile of palliative care in the Wimmera.
Key Responsibilities Provides competent nursing care that demonstrates the integration of advanced palliative care nursing knowledge and skills • Advocates for the health needs of clients/carers for their palliative care needs Contributes to the formal and informal palliative care education provided by Wimmera Hospice Care.
Ability to work effectively, both independently and as part of a multidisciplinary community health team and in collaboration with groups and individuals Skills & Experience Essential: Registered Nurse with Australian Health Professionals Registration Authority (AHPRA).
High level community-based experience Palliative care nursing experience Well-developed communication and interpersonal skills.
Demonstrated ability to work effectively, both independently and as part of a multi-disciplinary health team.
Current Victorian Drivers licence Desirable: Demonstrated and ongoing commitment to professional development.
Post graduate qualifications in palliative care or working towards same.
