Your Opportunity
Be a Game-Changer! Our Going Places Program is on the lookout for energetic and compassionate team members to assist individuals facing chronic homelessness or sleeping rough. Bring your expertise in case management with a strengths-based, 'housing first' approach and help guide people towards stable housing and a brighter future. Going Places is a multidisciplinary street to home program which combines clinical health services with coordinated case management to support people sleeping rough in Cairns to move into stable, long-term and sustainable housing
Key responsibilities will be to:
* Respond to referrals and conduct outreach across the community to connect with participants
* Build trust and rapport with our participants to encourage engagement in the program
* Build and develop strong, constructive relationships with agencies and community partners.
* Engage in creative strategies to support improved program access for people ‘sleeping rough’
* Support participants to appropriately engage with the Immediate Housing Response
* Update Case notes to ensure effective record keeping for the program
Requirements for Success
* Ideally qualifications in Community Work, Social Work, Psychology or Behavioural Sciences.
* Ideally some case management experience but not essential
* Proven experience building trust and developing relationships with participants and stakeholders.
* Some administration and record keeping experience
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
