Aged Care Case Manager - (Female designated)We began 23 years ago with a knock-out exhibition at the Art Gallery of NSW which raised over a million dollars. People from the remotest, most beautiful part of the Western Desert in Central Australia then used this money to develop their own organisation, with a culture of caring, compassion and determination.At the time, the focus was to get family members home to Kintore on dialysis. For people to be looked after on country staying connected to family and community, and to maintain the traditions of looking after country and passing on cultural heritage to future generations.It took a little time…and a lot of hard work…but in 2004, Purple House started dialysis in Kintore, the first of its kind, and set a precedence for many others to follow.Today we are proud to say that we have helped many communities to get their people home and now operate dialysis in 20 (and growing) communities across NT, SA and WA, with more on the way. We also run two mobile dialysis trucks, provide a range of support services including aged care, NDIS, primary health and social support, run a successful bush medicine social enterprise and are always working on new innovations.This is a Female Identified Position. Due to requisite cultural knowledge and sensitivities needed to perform this role (including personal care and grooming), it is a genuine occupational requirement under s35 of the Northern Territory Anti-Discrimination Act 1992 that applicants are female.POSITION SUMMARYPurple House supports Aged Care Services in Alice Springs, Yuendumu (YOPP) and Mount Liebig communities. Positions available in Alice Springs and Remote. Workers may be required to work and or live remote. The Case Manager is responsible for monitoring the individual needs of clients and their service provision. Key responsibilities include client documentation, comprehensive progress notes and liaison with carers and families. This is a hands-on position where issues can arise at short notice that require the Case Manager to be flexible, adaptable and resourceful in translating standards and improvement opportunities into best practice.STANDARD REQUIREMENTS Demonstrate a courteous and respectful manner and communicate effectively with internal and external stakeholders.Demonstrate cultural awareness and knowledge of cultural safety and Indigenous community control.Maintain confidentiality of information about clients, other staff members or community members.Work to achieve standards of best practice and promote continuous quality improvement.Demonstrate safe work practices and promote a safe work environment.FUNCTIONS Positively promote Aged Care and NDIS as Purple House services.Case management, care coordination and person-centred care that supports clients' rights, dignity and independence.Administration, reporting and compliance with consumer-directed care requirements, and Community Care National Standards.Accurate record keeping, report writing, and financial administration of the service budget.Operational support and maintenance of assets.Provide mentoring and training to Aged Care workers.Work collaboratively and maintain effective relationships with stakeholders.Promote a safe work environment in accordance with legislative requirements, adhere to Work Health Safety guidelines, and report all incidents and near misses.Other duties as required within trained capacity and level.KEY COMPETENCIES (SELECTION CRITERIA) Minimum Certificate III Aged Care or similar qualification (Community Services, Disability Work).Experience in staff maintaining and training.General knowledge of community aged care and key standards for service delivery and case management.Cultural sensitivity and ability to empathise with clients and advocate on their behalf.Sound administrative skills including report writing and computer literacy.High level skills in communication, conflict management, negotiation and problem solving.Proven high level of organisational skills and ability to multi-task.Willingness and capacity to work in a cross-cultural environment.Willingness to work under the direction of Indigenous leadership.Ability to work collaboratively in a multi-disciplinary environment.A knowledge and commitment to Quality Improvement.Knowledge of current Work Health Safety Legislation.Experience working with people with end-stage renal failure and a range of complex co-morbidities; including working in areas of aged care, chronic disease, musculoskeletal issues, and disability.Current National Criminal History check (less than 6 months old).Unrestricted driver license to be able to drive in the NT.Desirable:Experience working for an Aboriginal organisation and awareness of the issues pertaining to this region.Experience living and working in remote communities.4WD competency and experience driving on unsealed roads.About us - What we offer When required to live and work remote for more than 21 days, in either Yuendumu or in Mt.
