Job Description The Primary Mental Health Nurse will provide clinical and mental health support for people in detention with mental health presentations including complexity and risk, development of mental health recovery plans, medication management, referrals and discharge planning. KEY RESPONSIBILITIES Work in collaboration with the primary health care team to ensure a multidisciplinary, coordinated, evidence based and integrated approach to service delivery. Participate in the assessment, planning and implementation of care based on individual detainee needs incorporating a holistic, recovery orientated approach to mental health care and service delivery. Ensure participation in relevant training and professional development for supporting high quality nursing. Provide mental health interventions relating to the management of mental health symptoms, physical health monitoring whilst initiating treatment and therapeutic interventions, as clinically indicated. Contribute to the ongoing development and improvement of primary mental health services and mental health outcomes of detainees. Undertake the timely completion of mental health assessments including formulation, risk assessment, psychological treatment, crisis intervention, crisis management plans, outcome measures, documentation of progress in the electronic medical record system, discharge summaries and other required documentation and activities as required.
