Position Description:
An exciting opportunity now exists for a Diabetes Educator to join a Practice in Townsville. As part of a supportive, diverse, and multidisciplinary team, promote optimal health and wellbeing outcomes for individuals and communities at risk of or affected by diabetes. Working with non-acute clients, you will liaise with other Health Professionals to oversee the overall case management of clients. You will work with a diverse range of clients, set your schedule and have the chance to see the real difference your work makes as you enable rural communities to live and receive high-quality holistic care.
Core duties:
Integrate diabetes self-management education with clinical care
1. Utilise group education to assist with meeting individual needs
2. Determine comprehensive self-management plans to maximise health outcomes
3. Raise the awareness of healthy lifestyles, disease and disability and other health-related issues
The clinical caseload is varied, giving great exposure to a range of different clinical presentations, allowing you to explore all areas of your discipline early on in your career. They provide genuine community and not-for-profit primary health care services where there is no cost to the client. The Practice have a great team culture with many early career professionals across our business which allows for strong friendships to be formed and a wealth of peer support to be provided. Working with a large multidisciplinary team, you will learn about other health disciplines, holistic health care and referral pathways, allowing a wraparound service for your clients. At whatever level you join, you'll have ongoing mentorship and support and the ability to choose your own external mentor. Plus, you will receive up to 10 days of paid professional development leave per year, and up to $10,000 for outside professional development (including flights and accommodation if required).
Qualifications & Experience:
The right candidate will possess a degree or equivalent qualification in a health-related discipline and be able to provide high-quality diabetes assessment and develop an effective ongoing management plan. In addition, you will need:
4. Combined blue card & NDIS worker screening
5. Current, unrestricted ‘C’ class Australian Driver’s Licence
6. The right to live and work in Australia
7. Credentialing or ability to be credentialed with the Australian Diabetes Educators Association is essential.
