What you'll be doing
Reporting to the District Director Community, Wellbeing and Allied Health, the Director Community, Chronic and Complex Care provides strategic and operational leadership of the clinical services, programs and people that comprise the community portfolio and ensures the development and provision of high - quality health care services that meet accreditation standards and are patient and family centred. An important component of the role will be to better coordinate services between hospital, community, social care, aged and primary care settings.
The portfolio involves complex change management challenges to transform models of care and service partnerships between multiple providers. The Director will drive innovation across existing services and lead the development of new models of care to meet future demands by enhancing out of hospital service delivery.
Director Community, Chronic and Complex Care is responsible for setting future directions for the services in conjunction with the senior leadership team, Clinical Directors, other Facility Directors and District Executive. The Director will drive innovation across existing services and lead the design, development and implementation of new and novel models of care.
