Summary:
As a Clinical Care Coordinator, you will oversee the clinical team to ensure high-quality care while enhancing the quality of life for our residents. You will play a key role in maintaining compliance standards, mentoring your team, and ensuring seamless delivery of care services.
Responsibilities:
* Supervise the delivery of care services to meet safety, effectiveness, and regulatory standards.
* Lead, mentor, and support the nursing team, including managing schedules, conducting performance reviews, and fostering a positive work environment.
* Develop and review individualised care plans, collaborate with multidisciplinary teams, and address changes in resident needs promptly.
* Participate in quality assurance activities, conduct audits, and implement improvements to maintain high care standards.
* Ensure accurate and timely documentation of care plans, incidents, and resident records.
Key Requirements:
* Prior experience as a Clinical Care Coordinator (or similar leadership position) is highly desirable but not essential.
* Willingness to work over weekends is required.
* Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
* Excellent clinical knowledge, leadership, organisational, and communicative skills with the ability to lead, manage, and motivate a clinical team effectively.
* Strong knowledge and experience of aged care accreditation, regulations, standards, and best practices.
