As a Registered Nurse, you will take on the role of a Care Coordinator ensuring optimal outcomes for consumers who are affected by a severe and enduring mental illness and their carers through the promotion of best practice including the appropriate escalation of care.The successful incumbent will work collaboratively with members of the multidisciplinary health team in the planning, implementation, assessment, and evaluation of patient care.
