A confluence of factors including an ageing population; unprecedented hospital system pressures; national reforms across aged care, disability, and health; and changes in consumer expectations, all create risks and opportunities for our health system. We are looking for an experienced and positive leader to steer the Community Programs business unit during this transformational time.
The Community Programs team designs and delivers strategic programs at the interface of the hospital system and our aged care, disability and broader health and community service systems, including programs that support timely discharge from hospital. This includes (but is not limited to):
1. oversight and reform of significant joint Commonwealth/State aged care programs (, Transition Care Program, Aged Care Assessment Program)
2. designing and delivering innovative programs that support patients with a disability to safely leave hospital, (, )
3. co-designing pilots with Aboriginal Medical Services, as showcased in
4. a whole of Government Long Stay Patient Program including the WA Government response to the
The Director Community Programs will be able to deliver operational outcomes and reform, while utilising this knowledge to deliver strategic advice to Government. Given the cross-sectoral nature of the community portfolio, exceptional relationship skills will be crucial to build collaborative relations and influence outcomes across Governments and portfolios.
