The Care Partner is responsible for managing client registration, care planning and coordination of services to ensure the delivery of quality, client-centered customised care services. The Community team delivers consumer-directed care for older people including personal care, home nursing, domestic assistance and a range of other support services such as shopping, transport and respite.
