About this role
The NMHS Mental Health Older Adult Mental Health Program is responsible for the delivery of specialist mental health services to older people living in the North Metropolitan region of Perth. The Older Adult Program (OAP) operates several Older Adult Mental Health Services (OAMHS) that provide inpatient, Hospital in the Home and community services from locations in Wanneroo, Osborne Park and Shenton Park. A Geriatric Psychiatry Consultation Liaison Team also provides assessment and treatment planning to both Sir Charles Gairdner Hospital and Osborne Park Hospital.
The Hospital in the Home (HiTH) Service provides daily mental health care in the home of eligible consumers who would otherwise require admission to hospital. HiTH is a multidisciplinary team of mental health clinicians providing intensive mental health assessment, treatment and management provided in the home, enabling consumers to maintain their routines and fulfil their normal functioning and social roles, while still receiving the acute care they require. HiTH clinicians work in partnership with the consumer, their family or carers if consented to or where appropriate, for up to 14 days, making plans and providing interventions to support their recovery.
As part of a multidisciplinary team, the Senior Social Worker plans, implements, coordinates and evaluates Social Work services to HiTH consumers using advanced Social Work clinical practice skills, including case management. In this role, you will plan, implement and evaluate patient safety and quality of care. You will also supervise Social Work staff and students, provide support, coordinate duties and manage performance. You will practice as a Senior Social Worker and ensure practice is in accordance with current relevant social work professional practice standards and codes of ethics.
