In this role, you will exercise extended autonomy of decision making and clinical judgement while coordinating the flow and care coordination of patients in the palliative care service. This will include, but is not limited to, triaging referrals, managing waitlists, leading multidisciplinary meetings, collaborating with internal and external key stakeholders, and reviewing data for service improvement.The Palliative Care Clinical Nurse Navigator is an experienced registered nurse with advanced knowledge of the health needs of patients diagnosed with life-limiting illnesses. They will demonstrate the ability to act as a professional role model and clinical leader, with a high-level capacity for patient advocacy, while working independently and as part of the multidisciplinary team.About you:We are seeking a skilled and energeticClinical Nurse Specialistwho possesses a passion for person-centred care.
