About this role
This position provides advanced social work services to consumers of the Independent Community Living Strategy (ICLS) and the Community Mental Health Service. The role functions as a case manager undertaking clinical assessment of an allocated case load of consumers with a mental illness.
The Senior Social Worker will contribute to the development and monitoring of social work practice, initiate quality improvement practices and implement changes and improvement for the mental health service. The Senior Social Worker is an expert social work resource and will provide consultancy and education for the multidisciplinary teams and other agencies and stakeholders.
This role reports to the Service Manager.
What we are looking for
A highly capable and values-driven Senior Social Worker to join our ICLS team Osborne Park Community Mental Health Service. You will be a positive, resilient individual with experience in implementing a range of clinical social work interventions at an advanced level with adult consumers, including crisis intervention, family interventions, and group work within a community-based setting. Recent social work experience and ability to provide care coordination in a case management framework within a mental health setting is essential to the role.
