About the role -
We are looking for an outstanding, customer-centric Delivery Support Lead to manage our “care force” of Personal Care Workers (PCW), Homecare Workers (HCW) spanning Eastern Metro region of Melbourne.
Key accountabilities -
Will include but not limited to:
* Induction of newly hired PCWs and HCWs
* Supervision, support, mentoring and professional development of PCWs and HCWs
* Coaching staff to high quality consumer care as per the individual consumer care plan
* Clear and concise documentation of consumer outcomes
* Ensure staff recognise changes in a consumer’s health status and report appropriately
* Respond to service delivery issues and implement solutions to achieve the expected consumer outcomes
* Compliance with organisation and regulatory requirements
* Independently on board new Baptcare consumers
* Workforce planning and development
* Administration – maintain employee files, utilise risk-management systems, maintain databases etc
Key Selection Criteria –
* Demonstrated experience in the supervision of staff within the Community Aged Care sector
* Ability to achieve consumer focused outcomes
* Ability to apply various strategies to engage with and influence key stakeholders to achieve operational goals and targets
* Demonstrated ability to improve staff performance through coaching & mentoring
* Capacity to manage and escalate issues where risks are identified
* Ability to continuously improve service quality and consumer experience
Essential –
* Minimum Certificate III in any one of - Aged Care/Individual Support, Home and Community Care and Disability
* Current equivalent Level II First Aid certificate
* Own vehicle and current Australian Driver’s License
* Satisfactory Police Check
* Have or be willing to get an influenza vaccine
* Have evidence of Covid-19 Vaccine double vaccination at a minimum
Desirable –
* Registered Nurse, Endorsed Enrolled Nurse or Enrolled nurse
* Medication endorsement
* Experience in a similar role within the Community Aged Care sector
* Experience in supervision of a mobile workforce
