Based at our Federal Mills location, this position offers the chance to make a real difference by helping people develop the skills and confidence they need to secure and maintain long-term housing. As a Case Manager, you will work closely with clients to address the causes and impacts of homelessness, guiding them toward safe, stable, and affordable housing solutions. Your work will also involve collaboration with external partners to ensure a holistic approach to client care. The key objectives of this role are: Manage a caseload of clients across various program areas, providing outreach support through a case management model. Conduct assessments to identify client needs and risks, creating and regularly reviewing individualized case plans. Maintain clear and accurate case notes, and engage in professional development to enhance service delivery. Develop and sustain positive relationships with external stakeholders to facilitate coordinated support for clients. About You To be successful in the role, you will have: A relevant tertiary qualification in Social Work, Psychology, and/or related behavioural sciences at degree level with substantial experience; or associate diploma level with substantial experience in the relevant service stream, or less formal qualifications with specialised skills sufficient to perform at this level. Minimum of two years' experience in community services, with a solid understanding of strengths-based case management, public housing, homelessness, and renter's rights. Strong communication skills, both verbal and written, with proficiency in Microsoft software (Outlook, Word, Excel) and the ability to work independently. Current full Victorian Driver's Licence and experience working with diverse communities, including CALD, LGBTIQ+, and First Nations peoples, with knowledge of family violence issues.
