Through co-designed research, we are implementing a service dedicated to young adult stroke survivors. A pilot clinical hub has been established within Austin Health, geographically based at the Melbourne Brain Centre in Heidelberg, as well as via telehealth.About the role:A regular day will see you:Provide outpatient clinic services for young adult stroke survivorsLead Young Stroke Service multidisciplinary meetings and provide coordination of young stroke services for patients.Support the further development of systems, processes and connectivity that underpin the Young Stroke Service.About you:This role requires you to have:Current registration for practice with AHPRA (Australian Health Practitioner Regulation Agency) as a Physiotherapist.At least 5 years clinical experience in allied health working with stroke or chronic conditions.At least 2 years project management experience in a health-related field and/or experience with establishing new health care systems or innovating current health care systems in a patient-facing role.Demonstrable high-level patient management and communication skills.Experience in working with clinical networks and Health Departments.Experience with web-based systems and processes for electronic data capture in healthcare and knowledge of medical terminology.Experience in writing/developing data collection tools, clinical checklists, reports, protocols, standard operating procedures and policies.Demonstrable attention to detail and is strategic in approaches to problem solving.Working at Austin Health means enjoying a strong sense of purpose, engaging in meaningful work every day.
