About the Role
The District Nurse is a key health position that delivers valuable community based clinical nursing services by practicing and promoting safe, efficient and effective nursing care that reflects current standards of nursing practices. District Nurses assist consumers to achieve their optimal health goals by delivering consumer centred services, whilst working as part of a multidisciplinary team.
The District Nurse delivers services through the Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP), Post-Acute Care (PAC), Hospital in the Home (HITH), Home and Community Care Program (HACC-PYP), Department of Veteran Affairs (DVA) and National Disability Insurance Scheme (NDIS) and a number of private brokerage contracts.
Key Responsibilities
* Conducts thorough nursing assessments of consumers and their environments.
* Collaborates with consumers, carers, and care teams to plan and evaluate nursing care.
* Delivers safe, evidence-based nursing care aimed at achieving consumer goals.
* Prioritizes workload based on consumer needs and clinical urgency.
* Effectively manages unexpected clinical situations and consumer deterioration.
* Upholds accountability, seeks guidance when necessary, and respects cultural and psychosocial factors while practicing within legal and professional boundaries.
Skills & Experience
* Registered Nurse with current AHPRA registration and over one year of postgraduate experience.
* Proven experience in community, hospital, or acute care settings, with advanced clinical skills in wound care, continence care, and diabetes management.
* Strong assessment and consumer-centered care planning abilities aligned with wellness and reablement principles.
* Effective workload prioritization and performance under pressure.
* Excellent interpersonal communication and computer literacy skills.
* Self-motivated, able to work autonomously, and within a team; holds a current Australian Driver’s License.

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020 - 2024. The classification is District Nurse Level 1 (YF4) to District Nurse Level 2 (YU1) which ranges from $48.26 - $50.32 per hour.
