This team provides a range of population and patient centred strategies with the aim of supporting patients to be able to live independently in the community. The GEMRHITH (geriatric evaluation, management and rehabilitation Hospital in the home) is a geriatrician-led multidisciplinary service which provides care in the home through a hospital substitution model. The allied health component of care includes physiotherapy, occupational therapy, social work, nutrition and dietetics and speech pathology. We are the major public healthcare provider for Brisbane's south side, Logan, Redlands and the Scenic Rim. We operate five major hospitals and a range of community, specialty and state-wide healthcare services. We are renowned for teaching and research excellence. About the Role: The purpose of this role is to deliver senior social work services within the Community Services Team, GEMRHITH and MAPS model of care. This includes: Delivering high quality evidenced based social work clinical services, specialising in complex geriatric care to patients/consumers within a highly functioning interprofessional team. Contribution to the advancement of quality social work services and optimal patient care, through provision of service development, quality improvement, and clinical education. Monitoring and reporting on work practices and outcomes, including initiating, planning and evaluating relevant quality and research initiatives. Providing supervision and training to less experienced (HP3) clinicians (2x FTE), students and to Allied Health Assistants (AHA's).
