Support Coordinator/ Psychosocial Recovery Coach Role
We are seeking a dedicated Support Coordinator to join our team in the Sunshine Coast region, commencing immediately.
This part-time role involves collaborating with clients to optimize their NDIS-funded supports, connecting them with service providers and Allied Health professionals, and monitoring progress towards plan goals.
The successful candidate will have excellent interpersonal and communication skills, strong written communication abilities, and demonstrable problem-solving skills.
We are looking for someone with minimum 2 years experience in support coordination, familiarity with psychosocial disabilities, and relevant qualifications in Disability Services or Cert IV in Mental Health/Mental Health Peer Work.

Key Responsibilities:
* Assist clients and their families in accessing and coordinating services.
* Develop collaborative relationships with internal and external stakeholders.
* Facilitate implementation of all supports outlined in NDIS plans.
* Enhance clients' skills to coordinate support and engage in their communities.
* Empower clients with a comprehensive understanding of their plans.
Requirements:
* Excellent interpersonal and communication skills.
* Strong written communication abilities.
* Demonstrable problem-solving skills.
* Capacity to collaborate effectively.
* Understanding or knowledge of the NDIS.
* Minimum 2 years experience in support coordination.
* Familiarity with psychosocial disabilities.
* Current NDIS Worker Screening Check and Working with Children Check.
* Current First Aid and CPR Certification.
* Relevant qualifications in Disability Services or Cert IV in Mental Health/Mental Health Peer Work.
